This card will be downloaded and returned to you. Please post the Pet Information Form on your refrigerator OR in the kitchen drawer nearest to the refrigerator. We have to be able to get to the information should there be an emergency. Copies are stored in the company office and with the lead sitter. We do not carry Pet Information Forms on our person from sit to sit. Therefore, it is the guardian's responsibility to post this information on the refrigerator or place in the kitchen drawer nearest to the refrigerator. Do not leave blanks. The information is asked because it is needed. Thanks! |

Permisssion to treat: Guardian's electronic signature and e-mail indicates permission to transport the pet to the verterinarian/s listed. If the listed veterinarian/s are unavailable, or do not return calls or pages, the pet sitter with contact the 2nd listed choice. If the 2nd choice is unavailable, or does not return calls or pages, the pet will be rushed to The Veterinary Hospital at the University of PA. Electronic signature belows alerts veterinary staff members of the guardian's guarantee of payment for services, as well as permission granted to the veterinary staff to treat the pet in a matter that is best suited to the pet's condition/s, as well in a manner that will disallow prolonged suffering. |
